To request membership with the Empowerment Council please choose one of the following options:

1. EMAIL THE FORM DIRECTLY TO US ONLINE: You may use an email client (such as Microsoft Outlook or Mozilla Thunderbird) or save and attach the form to your out going email via any web based email (such as Gmail, Hotmail or Yahoo!)

If attaching the form to web based mail please include NEW MEMBERSHIP REQUEST in the email subject and send to: beth_jacob@camh.net (these instructions are also printed at the bottom of the on-line form)

2. PRINT THE FORM AND MAIL IT TO THE EMPOWERMENT COUNCIL: Select the Membership form link below, complete the form and select the print button at the bottom of the form. Alternatly you may simply print the form and fill it out by hand. Mailing instructions are included on the Membership form.
